We are incredibly grateful to be nominated for The Guide to Texas Best of Texas Awards. To be recognized by the community we are so proud to serve is an honor for our entire Bridge Hospice family.

Bridge Hospice is proudly locally owned and rooted in West Texas. Founded in 2019 by Tammie Ware, a healthcare professional with more than 25 years of experience in hospice, palliative, and elder care, Bridge was created with a vision to provide a more personal, compassionate approach to end-of-life care—one that puts patients and their families first.

That commitment has helped shape both our organization and hospice care in our community. In 2022, Tammie was honored by the Texas and New Mexico Hospice Organization with the prestigious Charley Wasson Hospice Advocate Award, recognizing her innovative programs and dedication to advancing patient advocacy in West Texas.

Among those efforts is the development of West Texas’s first Patient Advocate Program, created to better equip caregivers to support patients with physical or memory limitations. Tammie also helped establish Dave’s Wish, the nonprofit arm of Bridge Hospice, which directs 100% of its charitable funds toward helping hospice patients in Lubbock County with needs such as food, rent, utilities, transportation, clothing, and other expenses not covered by hospice.

Bridge Hospice also offers Valor, a specialized program dedicated to honoring and supporting veterans at the end of life. Through individualized recognition and compassionate care, Valor helps ensure veterans are honored for their service while receiving the dignity, respect, and support they deserve.

Today, our interdisciplinary team continues that same mission through individualized care centered on comfort, dignity, and quality of life. We believe hospice is about more than caring for a diagnosis—it is about caring for the person, supporting the family, and making the moments that remain meaningful.

West Texas is more than where we provide care. It is our home. We are deeply grateful for the trust our community places in us and honored to continue our mission of extending love, comfort, and care to local patients and families.
